Immunization Rates and Requirements
Oregon law requires that all children in Grades K-12 entering Oregon schools for the first time be immunized. New students will not be allowed to start school without the required immunizations.

DTaP Polio Varicella MMR Hepatitis B Hepatitis A Hib Tdap K or Grades 1 - 6 5 4 1 2 3 2 Grades 7 - 11 5 4 1 2 3 2 1 Grade 12 5 4 1 2 3 1 School administrators are required to exclude from school those students not having proof of at least one dose of each of the above-noted immunizations. The only exceptions are those children who have health problems that would prevent them from receiving the inoculations and those whose religious teachings oppose immunizations. Senate Bill 895, passed in 2015, requires schools and child care facilities to have their immunization and exemption rates available at their main offices, on their websites, and for parents on paper or electronic format.
